Patient had the root canal, post and crown done many years ago in another country. Our x-ray shows a large cyst growing at the end of the root. It is pressing on the root so much that it is causing the root to be 'eaten away'. We call it external root resorption. Re-doing the root canal will only destroy the post, crown, and the tooth itself, so the decision was made to do surgery (apicoectomy) to remove the cyst, clean, and seal the end of the root and at the same time preserve the crown and tooth.
This patient came in to our office to see me about a tooth that already had a root canal done many years ago. The crown was good and the root canal looks like it was well done by another endodontist, yet there is pain and swelling. X-ray taken showed a large circular lesion (dark area) around the ends of the two forward roots. Consider how well the previous root canal was done and the size of the lesion, I told patient that most likely it is a cyst. Wanting to preserve the good crown as well, I recommended that a surgery (apicoectomy) should be done to get rid of the cyst and at the same time saving the crown for the patient.
Pre-operative x-ray shows a good looking root canal, but also a large circular lesion around the ends of the forward roots. Patient is having symptoms of pain and swelling. Apicoectomy is recommended.
Immediate post surgical x-ray shows that the roots were cut by 2 millimeters, the canal openings were cleaned ultrasonically and sealed with MTA, the large cyst was removed, and a small amount of bone graft was placed to promote bone healing.
6 months after surgery, I brought patient back to check on the healing and found complete healing! The cyst is gone and new bone had grown in its place! I was able to preserve the tooth, the crown, and get rid of the cyst by doing the surgery.
